It was erected on 26 October 1962 from territory of the Diocese of Santa Marta. Its seat is St. Anne's Cathedral in Ocana.

History
Seated in the city of Ocaña within Colombia's ecclesiastical province of Nueva Pamplona, this diocese (Latin: Ocaniensis) was erected by Pope John XXIII on 26 October 1962, taking in territory from the departments of Norte de Santander and Cesar. Across its 18,000 km2 it embraces 18 towns along with two townships in the municipality of La Esperanza, whose head belongs to the Nueva Pamplona.
